Be sure to get tickets for each person who'll be attending with you and follow the protocol below:
- Masks are required
- CDC Social Distancing Guidelines will be followed (6ft. apart)
- Must show proof of vaccination; if not, must show proof of negative test within the last 72 hours
If you are experiencing any of the following symptoms, we ask you to join us online for worship. (Note: We will be taking temperatures at the door. If your temperature exceeds 100.4 F, you will be asked to go home)
Fever (over 100.4F) or chills – New loss of smell or taste
Cough
Muscle aches
Sore throat
Shortness of breath or difficulty
breathing
New or unusual headache
New onset of any gastrointestinal
Symptoms (such as nausea, vomiting, diarrhea, or loss of appetite)
***ATTN*** Have you been in contact with someone who has been diagnosed with or tested positive for Covid-19? If yes, we ask that you stay home and worship with us online.
Note: If any of these symptoms occur after completing this attendance form, we ask that you exercise integrity and wisdom and stay home. These questions will be asked again upon entering our in-person worship experience.
