Catholic Charities, The Hispanic Senior Center offers recreational and socialization activities for older Hispanic adults, while focusing on the cultural and linguistic needs of seniors within Greater Cleveland's Hispanic community.
Our center offers individuals the opportunity to be among friends, sharing a common language and cultural background.
What you will do:
Provide support services to Hispanic Seniors.
Provide referral and follow up services to appropriate programs as needed.
Make visits to clients and their families.
Transport seniors to medical appointments or other social service appointments.
Act as an advocate to Hispanic Seniors.
Complete and maintain appropriate documentation and paperwork as required.
Compile data and information needed for program reports
Experience
Required
- 1 - 2 years: Case Management

- High School or better
