Program Statement:
Community-Based Family Services provides home-based mental health assessment and counseling to children and their families to sustain youth in the community with therapeutic support.
What you will do:
Provide clinical case management in the homes of youth who need more comprehensive and trauma-informed services
Manage a smaller caseload of youth and families with a need for regular contact throughout the week
Assist clients in developing formal and informal community support.
Provide clients with direct service, either in the office or in the field.
Review and monitor services, reassess needs, and initiate services responsive to clients' needs; provide service coordination.
Safety Planning and skill development to sustain you in their homes without disruption
Experience
Preferred
1 - 2 years: working with families and youth
1 - 2 years: working in the social services field
Education

Bachelor or better in Social Work
