We are seeking a compassionate person to assist our clientele as they transition from a nursing home to a new community-based living opportunity. In this role you would be able to utilize these skills and more!

**This position serves the following counties, therefore, due to required travel, the ideal candidate will reside in this general area: Cuyahoga, Lake, Geauga Stark, and Summit Counties**

Qualified candidates for this position offer:

  • High school diploma or GED required. Bachelor’s Degree in social work or a related field preferred.
  • Minimum of 1 years in social service delivery with the elderly and/or family populations. 2-4 years of experience preferred. Demonstrated working knowledge of supportive services and other resources in the area served by the project.
