Responsibilities
Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Middlesex County
Provides in-home counseling to CP&P referred families.
Case management and case documentation
Case conference with CP&P and other providers
Knowledge and experience in CBT and Motivational Interviewing preferred
Requirements
Rotating 24/7 coverage required
Master’s Degree in Social Work or related field with licensure as applicable to degree
Flexible schedule including availability on nights
Driver’s license needed with an acceptable driving history
Work Schedule: Full-Time
