The Community Liaison/Outreach worker will serve our communities by providing education, outreach, and gathering contacts that will evolve our organization's delivery of services, so that we serve our community more effectively and efficiently.

RESPONSIBILITIES:
Work closely with the VP of Integrated Health Services to develop new Partnership leads to connect clients with opioids and other addictions to access treatment services. Enroll/refer a minimum of 30 clients per month into Chemical Dependency and Mental Health Clinics.
Reach out to community places of worship, schools, hospitals, senior centers and other Community Based Organizations to connect clients with opioids and other addictions to access treatment services.
Assist clients when needed with applying for Medicaid, housing and connect clients with a Health Home Care Management services.
Independently manage day-to-day working relationship with external partners including consistent field visits, effective email correspondence and monthly feedback loop that tracks referrals and troubleshoots customer service issues with a sense of urgency.
Organize, adapt, and deliver effective presentations/patient education materials about opioids and other substances.
Deliver excellent internal and external customer service with the ability to problem-solve swiftly and create sustainable solutions that improve the clients' experience.
Develop strategies to meet enrollment targets and with the ability to execute client and community engagement activities that are compelling and successful with effective follow-through.
Maintain detailed knowledge of HeartShare's program and actively seeks out opportunities to learn and develop business skills in a human service setting.
Provide timely, accurate and high-quality written communication including referral documentation, weekly/monthly tracking reports, enrollment forecasts, agendas/meeting preparation, email follow-ups from meetings and other forms of communication as required
Adhere to all government and funder regulations.
Abide by all Heart Share St. Vincent Services policies and procedures.
Represent HeartShare St. Vincent Services, both within and outside the organization, in a manner that promotes the Mission, Vision, Values and Goals of HeartShare St. Vincent.
Undertake any additional tasks and initiatives as assigned by the Program Director or Senior Director.
Develop systematic assessment that can measure success of program's goals and objectives.

QUALIFICATIONS:
Bachelor's Degree in health care administration, business administration or relative field
A minimum of one year in outreach, sales, business development and or administrative field

CASAC preferred

Developed knowledge of social services organizations and other community-based services with established relationships
Must be results-orientated and manage time and resources to deliver in a high-performance setting
Computer literate with proficiency in Microsoft Office Suite including Excel, PowerPoint
Ability to work collaboratively in a team setting but is self-motivated and takes ownership and accountability over Partner relationships
Effective in written communication, public speaking/presentations and interpersonal skills
Must be able to read, write and speak English.
Bilingual in Spanish a plus.
