Primary Purpose:
Increases the capacity of highly vulnerable refugees facing barriers to access the tools, information and services they need to achieve lasting self-sufficiency. The position provides intensive case management and service coordination services for refugees with identified special needs including those with physical and mental health care needs as well as vulnerable populations such as single parents and the elderly. This includes the coordination of community workshops, and educational seminars.

Essential Duties:
Welcome and orient refugees to the community and culture.

Coordinates services with the case management team to determine new arrival case enrollment into the Preferred Communities program based on specific need for intensive case management services.

Creates comprehensive case plan with client and works together with client, R&P case manager and employment team to assist client to attain self-sufficiency. Work with clients to remove barriers to employment and realize the goal of family self-sufficiency. Connect clients to in-house support and make referrals to external service providers. Identify support services that may assist clients in becoming self-sufficient, such as ESL, transportation, immigration services, and childcare.

Together with case management team, review incoming medical cases, create initial care plan and make appropriate contacts and appointments as necessary prior to arrival in US.

Develop and nurture relationships with physical and mental health providers at the local level to increase access to health care for refugees.

Provide technical assistance and cultural sensitivity training to providers to build their capacity to serve unique populations.

Explore and/or develop innovative programs and opportunities to assist families with special circumstances such as those with single parent households to find stability and employment (e.g. child care cooperatives, ride shares, etc.)

Provide referrals for clients needing assistance with applications and follow up for eligible social services including SSI and Disability benefits where applicable.

Organize workshops and seminars for newly arrived refugees that provide information and tools to assist in transition in areas related to stress management, culture shock and healthy relationships and families.

Advocate on behalf of refugees and refugee-serving voluntary agencies.

Offer interpretation and translation services to refugee clients, as well as transportation to interviews and appointments as necessary. (Compensation is paid by CWS for mileage when using personal vehicle.)

Document all contacts and services in client case files and maintain case note logs. Complete all required reports.

Attend trainings and conferences as requested by supervisor.

Keep informed about refugee and immigrant issues, and available services to refugees, using current information to better identify and serve refugee clients.

Represent the program at meetings and speaking engagements relevant to the responsibilities of the position.

Perform all duties in a culturally and linguistically appropriate manner.

Undertake other duties as assigned.

Work varied hours, including some night and weekend work.

Must have valid driver’s license and suitable vehicle for transporting clients.

Qualifications:
High school diploma or equivalent and two years of work experience. Bachelor’s degree preferred.

Ability to work in a multi-cultural environment required.

Experience: The successful candidate must have a minimum of one year’s previous work experience in the field of social work, public health, mental health services and/or immigrant communities. Cultural sensitivity and fluency in English are essential. Bilingual language (Ukrainian or Russian) strongly preferred.
