ESSENTIAL DUTIES:
Implementation of the program in accordance with contract requirements, completion of Family Connections core training and DCS Standards of Practice.
Family Connection Consultant will conduct home visits and maintain case load as assigned.
Complete individual home visits at a minimum once a week and deliver FC Curriculum.
Prepare for home visits and complete documentation of visit within (1) business day of completion of the visit.
Conduct home visits at time that is convenient for the family. This may include evenings and weekends.
Conducts comprehensive family assessment and other assessments as required with families that accept services according to the Family Connections Model and contract guidelines. Works with DCS Specialist to engage and assess the family.
With participation of the family, develops and implements a Service Plan ensuring UBSMART goals, and the interventions that will support family members to achieve outcomes and goals.
Maintain case records and data system in accordance with agency and program policies, procedures, and requirements.
Initiate contact and conduct a home visit with referred families within one (1) business day of referral and actively work to engage families in the program.
Attend mandated virtual or in person trainings, conferences, meetings or community events, which may include out-of-state travel.
Connect families to resources that address their needs.
Maintain a positive working relationship with colleagues, administration, collaborative partners and others within the community.
Participate in a one-hour individual supervision and one hour group supervision per week.
Makes appropriate referrals to government & community agencies as needed. Make appropriate referrals to sub-contractors within the program using established procedures & forms. Follows up with sub-contractors regarding progress.
Report suspected child abuse or neglect to the Department of Child Safety (DCS).
Complete training in the Nurturing Parenting Program practice model.
Utilize or conduct intakes per the Nurturing Parenting model.
Collaborate with the parent/caregiver to gather information and complete the Family Nurturing Plan (FNP) that identifies the parenting constructs and competencies that will be the focus of NPP services.
Conducts a minimum of two hour weekly in person parenting sessions with family and documents family interaction and progress in a timely manner.
Schedule a mid-point session with the family, to occur halfway through the service request period, to evaluate the parent/caregiver’s progress towards achieving the NNP goals.

Close case when:
targeted UBSMART goals have been addressed;
the client requests case closure;
provision of service can no longer achieve goals; or
when identified reason(s) have been met per the Nurturing Program Manual.
Complete documentation required for case closure.

22.Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.

23.Performs other duties as assigned.

JOB REQUIREMENTS:
Education /Background: Minimum of a Bachelor’s Degree in Social Work or related field with at least one (1) year of relevant experience.
Job Knowledge: Must have knowledge of community resources, case management, assessment and service planning, and problem solving with complex family needs. Understanding of DCS systems preferred. Must be able to work under pressure; must be able to meet deadlines and set priorities; must be well organized and able to effectively manage multiple cases; must be able to work independently; must be able to work independently and as part of a team; must be flexible and able to adapt to changing working conditions. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications. Intermediate-level of technology and software use to complete documentation and case work.
Working Conditions/Physical Demands: Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. Requires considerable work in off-site home environment where Catholic Charities has no control over the accessibility or condition of these homes. Requires willingness to be on-call for emergencies. Position requires evening and weekends; may lift up to 25 pounds.

Other requirements:
Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
You must be 21 years of age or older to drive on behalf of Catholic Charities.
Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.
