POSITION SUMMARY:

The Caseworker Coach IV-Rapid Rehousing Coordinator, under the supervision of the Director of Homeless Services, serves individuals and families referred through the Coordinated Entry Systems in San Diego and Imperial Counties. This team member is responsible for assisting individuals and families with permanent housing placements within 30 days of intake. Other core services include mitigating housing barriers, mediating tenant/landlord issues, relocating households who are unable to maintain their placements, identifying potential vacancies, and building a landlord network.

ESSENTIAL FUNCTIONS:

  • Initiates contact with clients referred to Rapid Rehousing through the Coordinated Entry Systems in San Diego and Imperial Counties
  • Performs intake interviews, assesses clients’ strengths and barriers, and refers clients to appropriate community resources, as needed.
  • Assists clients with obtaining and completing necessary documents to access housing, benefits, or increase their income
  • Works with clients to identify and overcome barriers to securing housing.
  • Advocates with landlords to overcome clients’ challenges in securing suitable housing.
  • Assists clients with completing and submitting rental applications, paying application fees, following up with application status, as well as submitting appeals if their rental application is denied.
  • Educates clients on the current housing market, housing search options, fair housing laws, and tenant rights.
  • Ensures housing placements meet all HUD requirements: habitability inspection, fair market rent, reasonable lease terms, etc.
  • Assesses the client's needs, and arranges, coordinates, monitors, evaluates, and advocates for a variety of services to meet those complex needs and responds to unanticipated client/resident needs as they arise.
  • Assists with transportation and moving clients into their residences, securing furniture, supplies, and other housing essentials.
  • Participates in multi-disciplinary client review team meetings and develops action plans that address each client’s individual needs.
  • Establishes and maintains positive relationships with landlords and rental agencies offering affordable rental properties
  • Provides case management and supportive services for clients after they are housed to support them in maintaining their housing and stability.
  • Mediates disputes between clients/tenants and landlords.
  • Documents all client contacts in Clarity HMIS and Catholic Charities tracking systems consistent with established community standards and Agency requirements.

 

 

QUALIFICATIONS:

  • Minimum of two (2) years of experience providing case management for people who are unhoused.
  • Minimum one year of experience serving people living with mental illness and/or substance use disorders.
  • Associate’s degree in social work, psychology, counseling, or a related field or equivalent experience
  • Working knowledge of best practices including Housing First, Low Barrier Services, Trauma-Informed Care, Motivational Interviewing, Diversion, Homeless Prevention, Progressive Engagement, and Conflict Resolution
  • Well-developed problem-solving skills and flexible, creative thinking skills with a positive, can-do attitude
  • Valid driver’s license and access to reliable transportation. This position travels between San Diego and Imperial Counties.
  • Bilingual Spanish speaker preferred.
  • Strong working knowledge of tenant’s rights and responsibilities.
  • Excellent communication skills including listening and writing skills.
  • Possess strong organizational skills and the ability to multitask and meet a demanding workload.
  • Detail oriented and able to comprehend Rapid Rehousing Community Standards and funding regulations and apply them to each client’s unique circumstances.
  • Familiarity with or ability to quickly learn the resources and key services providers (governmental, civic, social, educational, economic, faith-based, etc.) available to help clients quickly overcome barriers and secure housing.
  • Act as a liaison with partner agencies.
  • Ability to work independently with minimal daily supervision required.
  • Experience with data collection and report preparation, ability to interpret and clearly summarize data to reflect program outcomes, HMIS experience preferred. Candidate must be able to successfully complete HMIS training and obtain a Clarify user license.
  • Strong computer skills. Ability to read and send e-mail and to utilize MS Word, Excel, ACCESS, and PowerPoint.
  • Ability to maintain the confidentiality of current and former clients, landlords, and other matters affecting tenant relations.
