Position Summary:
San Ysidro Health (SYHealth)’s Research & Health Promotion Department enhances SYHealth’s clinical care with health promotion, research, and care coordination programs for patients and community members. Under the general direction of the Director of the Research and Health Promotion Department, the Program Manager for Health Support Services will be responsible for the implementation and all operations aspects of the core health support services: Care Coordination, Health Education, Outreach, and special projects, such as Project Dulce, Diabetes Prevention Program, quality initiatives, Medical Group visits (operations funded) and the non-operations/grant-funded programs (e.g. LUNItA, Project Dulce, LUNA-E, etc.) of the department. The Program Manager will also be responsible for program development, conducting literature reviews & needs assessments, development & implementation of health promotion programs and projects, and program evaluation; grant proposal identification, development, and submission; grant management, including attention to budgets and personnel supervision; communication and reports to funding agencies; the recruitment, hiring, and supervision of department staff; and the facilitation of program integration across SYHealth clinical delivery sites. Under the guidance of the department’s Director, the Program Manager will be responsible for preparing operational analyses and reports, performing audits on current procedures to monitor and improve the efficiency of operations (tracking medical appointments and patient compliance); development, implementation, and monitoring of policies and procedures (e.g., program standard procedures, SYHealth local procedures), QA/QC and Data Management, participant engagement, community presentations, and retention strategies using social media activities, and expansion of operation funded programs within the SYHealth clinic network. The Program Manager will serve as a role model for cultural and linguistic competency for other staff to emulate and will maintain confidentiality at all levels.

Essential Functions of the Job:

  • Develop excellence in providing services that enrich and enhance the quality of life, dignity, and respect of SYHealth patients and those interested in the clinical and non-research programs in the department.
  • Responsible for general oversight and administration of the Care Coordination, Health Education Programs, and Outreach efforts which include:
  • Conducts program development, implementation, and evaluation.
  • Oversees the recruitment & enrollment processes, operational analyses, and reporting QA/QC measures,
  • Creates and refines data tracking system to monitor staff productivity and patient compliance.
  • Planning and evaluating the programs’ expansion within the SYHealth clinic network (i.e., West, Central, and East regions).
  • Conducts research & literature reviews about topics of interest (e.g., COVID-19, Diabetes Care); stays up-to-date on best practices
  • Coordinates health promotion/education and community service aspects.
  • Provides direct supervision, training, and evaluation of Program Supervisor(s), Care Coordinators, Health Educators, outreach coordinator, and other positions under its umbrella. Also, selects and assigns department staff within currently funded programs.
  • Responsible for supervision and evaluation of staff performance through annual performance appraisals. Evaluate performance and initiate or make recommendations for personnel actions.
  • Independently establishes, implements, and updates (as needed) protocols, policies, and efficient procedures for the department, including staff training/development, program management, and reporting; maintains quality assurance program standards.
  • Coordinates quality improvement and evaluation activities for contract compliance and ensures excellence in service delivery and adequate progress toward contract goals and objectives.
  • Works with the Director and other Program Managers in the Research/Health promotion Department and other departments in ensuring integration of and collaboration between Operations, Quality Management, Clinic Site Managers, and Ambulatory Care.
  • Works with the Director and Clinic Site Managers in increasing relationships and communication with other SYHealth programs and services to increase visibility and access to the operations and other programs in Health Promotion Department.
  • Support Program Supervisors in staff supervision and evaluation.
  • Ensure completion and review of specific monthly, quarterly, and annual reports, as required by the Director for the NCQA-PCMH yearly audit and/or SYHealth Executive Management. Work closely with Quality Management Department to monitor UDS report (productivity), HEDIS, and other quality of patient care measures every quarter.
  • Participates in and coordinates the design and implementation of data collection and analysis systems for funded programs and services, including computer and/or network systems, applications programs, and administrative procedures. Specifically, developing or refining a data tracking system to monitor staff productivity (medical appointments, relevant visits, etc.) and patient compliance in diabetes care performance measures (scorecard).

Fiscal Responsibilities

  • Works directly with the Director and in collaboration with other departments and managers to monitor budget expenditures and ensure compliance with specific budgets and funding sources.
  • Monitors and authorizes program expenses in the absence of the Department Director.
  • Works cooperatively with accounting and contracts departments to monitor monthly grant expenses. Oversees budget forecasting, contract monitoring, and review salaries expenditures.

Additional Duties and Responsibilities:

  • Perform other related activities as required or assigned by the Director of the Research and Health Promotion Department, the VP of Health Support Services, or the Chief Medical Officer.
  • Assist in identifying grant funding opportunities related to the Department’s services and programs; work with the Director and Program Managers to review grant proposals and budgets before submission (if applicable).
  • Ensure that program procedures are HIPAA compliant and report any deviations in accordance with NCQA-PCMH guidelines.
  • Integrates Total Quality Management (TQM) activities within research projects and service programs where possible, (e.g., research projects that collect data on SYHC could be used for TQM reports).
  • Establishes and maintains positive and informative relationships with appropriate local medical, civic, governmental, social service, and health promotion community groups.
  • Enhances professional growth and development among staff through participation in educational programs, current literature, in-service meetings, and workshops.
  • Represent SYHealth’s Research and Health Promotion Department to other organizations and community groups.
  • Ensure all records are kept, and activities are conducted in accordance with SYHealth and other agencies’ Policies and Procedures.
  • Adheres to SYHealth’s and department’s attendance and punctuality policies and practices.

Job Requirements

Education Required:
Advanced Degree: Master’s (e.g., MPH, MA, MS), Ph.D., MD (both U.S. and international degrees are acceptable); or:

  • Bachelors (e.g., BA, BS), with six years of related/progressive experience; or,
  • Associates with 8 years of relevant experience; or,
  • High School Diploma with 10 years of relevant experience

Experience Required:
Minimum three years of supervisory experience implementing programs in a non-profit health or human service agency or another health setting or equivalent. Minimum three years of experience working with clinical or patient-centered care programs (e.g., PCMH) or equivalent.

Certifications Required:
CA driver’s license with appropriate insurance coverage.

Verbal and Written Skills Required to Perform the Job:
Excellent interpersonal, written, and verbal communication skills.
Ability to convey ideas and information to others and receive feedback effectively.
Bilingual (Spanish or Arabic/Aramaic or Tagalog) preferred.

Technical Knowledge and Skills Required to Perform the Job:
Experienced in management, supervision, program development & evaluation

General office Equipment to include Personal Computer, Phones, Fax, etc.

Working Conditions and Physical Requirements:
Prolonged sitting position. Traveling within San Diego County; the position may require significant local traveling (including rural areas). Must have a car, CA driver’s license & insurance.
