The InnerCity Collaborative Case Management Model is a unique case management innovation geared toward implementing customized strategies for improving the lives of persons, individuals, and families directly impacted by violence in their communities.

The Case Manager shall use a trauma-informed clinical approach to develop and track individual service/treatment plans for each resident addressing client needs, i.e., connection to medical and mental health services, accessing public benefits/resources, budgeting, permanent and stable housing, and other resources to implement tasks, provide structure, give directives, and intervention to assist residents in meeting their treatment plan goals. The Case Manager will work as follows: Monday – Saturday, 9:00 am – 9:00 pm.

During these hours, Case Management is expected to engage with the tiered individuals directly, and collateral contacts or perform duties and tasks that contribute to an individual’s Outcome Plan. After working hours and in the event of an incident, Case Managers are expected to contact the relative Violence Intervention Managers within 24 hours of receipt of the notice.

Case Managers are expected to attend Critical Incident Response meetings as assigned. The ideal Case Manager candidate must have related experience working with complex communities affected by poverty, trauma, and violence and must have excellent knowledge of policies and protocols that govern violence interruption, ceasefires, and mediations.

The case manager manages full-time caseloads of up to 20 individuals. Individuals are referred to the Case Manager by ONSE Program Managers Case Manager is responsible for working with high-risk individuals and all collaborative sources to develop service plans, identify and track data points, and regularly document individual outcomes.

The Case Manager is responsible for building positive relationships with priority community members and counseling and providing crisis intervention for referred individuals, including assessing and treating emotional and behavioral problems. The case Manager identifies individual needs and strengths, connects individuals to services and resources, identifies opportunities for personal growth, and provides a plan to be jointly implemented by Case Management Provider, ONSE Program staff, and referred individuals.

Other Case Management duties include but are not limited to: Providing receipt of referral to ONSE within 24 hours or one business day of receiving the referral. Participates in daily huddles to assess needs and coordinate services (as needed). Provide therapeutic services to patients and their families. Work in collaboration with multidisciplinary team members, including external partners. Completes required documentation and paperwork, including intake form, outcome plan, necessary assessments, and treatment planning.

Additional Skill/Qualifications :

• Bachelor’s degree or equivalent work experience. Preference is given ta an o Bachelor of Social Work, Psychology, or Human Service Administration degree from an accredited college or university. LCSW stat is is is is is preferable.

• Minimum 2 years of relevant human service, social services, or case management experience.

• Experience with Homeless Management Information System (HMIS) or comparable system required.

• Thorough knowledge of standard case management documentation for agency reporting and record keeping.

• Competency in essential community engagement, medical, social, and practical case management.

• Experience working directly with LGBTQ young adults is helpful but not required.

• Experience in transitional living program and facilitation preferred.

• Knowledge of substance abuse/recovery work, family systems, mental health concerns, and suicidality.

• Must be a self-starter who can juggle multiple tasks and needs with the ability to prioritize and execute tasks efficiently.

• Sensitivity to the needs and challenges of gun violence young adults/residents experiencing homelessness, at risk for homelessness, and who have experienced trauma.

• Thorough working knowledge of community (Washington, DC metro area) providers and resources, including but not limited to healthcare and mental healthcare, housing, employment, social and need-based services, transportation, and education resources.

• Excellent interpersonal and written communication skills.

• Experience with Motivational Interviewing preferred.

• Must be able to exercise patience and understanding even in escalated crises.

• Other duties as assigned by Clinical and Administrative leadership.
