Hiring Salary Range: $20.30-24.75

Essential Functions:

  • Provide specific types of case management services for the Elderly Waiver case management clients
  • Provide case management services that help support our licensed social workers by calling new clients, their clinics and other resource referral and collaboration with other providers involved in client care
  • Phone calls to encourage client involvement in our services and supports
  • Provide a supportive relationship to foster trusting relationships
  • Participate in team and program staff meetings
  • Complete daily documentation in electronic record systems as required by program and contract
  • Be supported by a professional team of administrative assistants who will assist in a wide range of activities

 


Minimum Qualifications:

  • Associates degree in human services or related field required and two years of experience working with clients.
  • In lieu of degree, a minimum of five years of experience working with clients.
  • Prefer experience in case management and/or knowledge of waiver services.
  • Ability to work with diverse and/or low income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness.
