Hiring Salary Range: $49,000-$59,000

 


Premium for second language proficiency

Essential Functions:

  • Provide case management services for Elderly Waiver or Disability Waiver case management clients
  • Provide case management services including assessments, care planning, resource sharing and referrals for services with clients in accordance with contract requirements and DHS policy
  • Provide a support to clients to foster trusting relationships
  • Participate in team and program staff meetings
  • Complete daily documentation in electronic record systems as required by program and contract
  • Ability to manage multiple demands on your time in a fast paced, ever-changing environment.
  • Ability to recognize the effects of poverty, social determinants of health, physical disabilities, mental health disabilities and trauma have on the individuals served in a compassionate, trauma informed manner.
  • Be supported by a professional team of administrative assistants who will assist in a wide range of activities

 


Minimum Qualifications:

  • Bachelor’s Degree in Social Work, Psychology, Human Services or related field is preferred.
  • Minimum of 1 year of experience working with older adults, people with disabilities and/or people experiencing ongoing impact of poverty.
  • In lieu of degree, a minimum of five (5) years of experience working with a specific client group is required
