The Registered Nurse (RN) Care Coordinator is responsible for the provision of care coordination to meet the physical and emotional needs of Catholic Charities clients. The RN Care Coordinator will demonstrate support for the Catholic Charities mission to serve those most in need, supporting a diverse population with a person-centered approach that emphasizes collaboration, dignity and respect. Responsibilities include supporting clients to meet their health and program goals and achieve health stability; educating clients about their health and wellbeing; educating other staff about medication assistance and monitoring; and ensuring quality care is provided or coordinated and arranged. Effective communication around coordination of care with other Catholic Charities staff and community partners is a key deliverable for success in this role.

ESSENTIAL FUNCTIONS

Provide care coordination and referral to ensure seamless delivery of care, improve communication among health and human service providers, and guide clients and communities toward an effective use of resources.

Provide support to patients, including health assessment and screening, education, coaching, medication monitoring, assistance and education, and ensuring the fulfillment of orders of the primary care or other integrated care providers.

Conduct initial and ongoing health assessments to determine clients’ physical, social and behavioral health care needs and to assist other providers in meeting these needs.

Work with clients to achieve program goals in a timely manner.

Maintain relationships with referring organizations and offer regular trainings on the role of medical respite care to appropriate staff from referring organizations.

Prepare individualized care plans and ensure at orientation that the clients understand medical respite, including program goals.

Facilitate preventive care measures. Offer health education, guidance, and motivational counseling to clients to encourage self-care and assist in decision-making.

Work with other Catholic Charities staff and community partners to facilitate the integration of health care and social services. Share information with other members of the interdisciplinary care team to effectively coordinate services and facilitate progress toward goal attainment.

Develop and maintain appropriate written protocols (e.g., for infection control or the appropriate transfer of client information) and a quality improvement plan.

Initiate and lead targeted “in-reach” at assigned site(s) to engage clients and their families and educate, refer, and connect them with community resources.

Responsible for promptly and accurately collect, enter, manage and report client data into systems of record.

Maintain up-to-date medication trainings and provide medication training (and test) the housing staff who offer medication assistance.

Other duties as assigned.

MINIMUM QUALIFICATIONS/TECHNICAL EXPERTISE

  • Current MN license for Registered Nurse.
  • Two to four years of direct nursing experience; Public Health Nursing Certificate preferred.
  • Experience working with underserved or vulnerable populations, including persons from diverse racial, ethnic, and economic backgrounds, as well as those with mental health and/or behavioral issues.
  • Demonstrated ability to work with diverse and/or low income populations, including those experiencing homelessness, unstable housing, and/or mental illness.

Job Type: Part-time

Pay: $32.40 - $39.60 per hour
