ACCOUNTABILITY

The responsibilities of this position are to be conducted under supervision of the Church Program Administrator of The Homeless and Creative Housing Division. The Program Administrator, along with the Director of Homeless Services, will provide oversight and guidance and will conduct requisite reviews as to job performance. This position is subject to the terms and conditions of the most current Personnel Policies and Practices Manual.

Any person assuming this position will have read and agreed to be subject to the terms and conditions of the Personnel Policies and Practices Manual of the First Presbyterian Church of Hayward, including an understanding that any employment agreement will depend upon a satisfactory background check and a satisfactory explanation of irregularities. The background check may include but is not limited to clearances by a background search agency to review for criminal records, education verification, Social Security trace, license or credential verification, previous employment verification, previous credit, DMV, etc.

Note that FPCH stands for First Presbyterian Church of Hayward, and it includes the church plants and other activities that are all under the same tax ID number. SHP stands for South Hayward Parish.

RESPONSIBILITIES

Case Manager

  • Provide case management at FPCH Resource Center, Winter Shelter and SHP
  • Provide case management services to unsheltered individuals. Case management includes meeting with individuals to verify and understand: (1) the need for housing; (2) status of housing history; (3) identification of any existing housing alternatives; (4) assessment of client’s ability to sustain housing, e.g. issues of mental health, stability and amount of income, employment readiness if unemployed or underemployed, and credit history; (4) individual’s and families’ financial assets.
  • As a step in the initial engagement with each unsheltered individual or family the case manager will refer them to Abode Services, Inc. (“Abode”) for complete intakes that are to be entered into the countywide Homeless Management Information System and for assessment using the Coordinated Entry System (CES). This referral is to be documented by email to the Abode Services homeless outreach coordinator, copying the Executive Director and the appropriate Human Services Manager and Housing Program Manager in the city in which the unsheltered individual or family is when initially interviewed.
  • The Case manager will assist unsheltered individuals to successfully connect with resources and services of government agencies and other non-profits, including Federal, state, and local benefits
  • Following CES evaluation by Abode Services, the case manager will consult the Executive Director regarding provision of motel vouchers to unsheltered individuals
  • Other duties as assigned

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee may be required to sit at a desk and use the computer and phone, as well as work on various administrative tasks. Repetitive use of hands and vision is required to do various functions. Reading, writing, and advanced math abilities are required.

Flexibility, speed and the ability to multitask and work under deadlines is required in order to meet job demands. Ability to stand for long periods of time, lifting, bending and flexibility of hands may be required to do various functions. Ability to hear and communicate well is required in order to help unsheltered guests, volunteers and office Staff.

TERMS OF EMPLOYMENT

This position is considered full-time non-exempt employment, for an average of 40 hours per week, although more or fewer hours could be necessary at certain times and will be arranged with the Program Administrator. Vacation, sick leave and other terms of employment shall be as provided per the Personnel Policies and Practices Manual of Session.

Pay: $23.00 - $26.00 per hour
