Position Summary: This position has primary responsibility for providing clinical case management services to CCEB’s trauma treatment program for youth, Experience HOPE, which provides culturally relevant, trauma-informed mental health services to students and their families in the Oakland Unified and West Contra Costa Unified School Districts. The primary goal of the program is to help youth navigate challenges in school and their lives. Referrals will come from the local schools Coordination of Services Team (COST) and may also include youth who have experienced the homicide of a loved one. EH Clinical Case Manager will carry a caseload of 12 to 15 clients, facilitate 1 Tier 3 Treatment and 1-2 Tier 2 Restorative Practices Circle Groups. Clinical Case Managers will deliver evidence-based trauma treatment to youth and families and be required to maintain fidelity to program and treatment models.
The Clinical Case Manager will perform all clinical and case management responsibilities according to best practice standards defined by the Council on Accreditation and Catholic Charities East Bay.

Responsibilities:

Learn, adhere to, and deliver evidence-based treatment models

Establish and maintain effective working relationships with school personnel, parents, and community members.

Establish and maintain effective working relationships with a variety of clients from diverse cultural backgrounds.

Conduct home visits with students and their families as appropriate
-
Partner with school staff, CBOs, and government agencies on needs of clients and their families.

Adhere to agency and program data collection requirements

Conduct services in homes, Catholic Charities offices, or wherever is needed as determined by client needs including use of Telehealth.

Deliver treatment services that best meet the immediate need of clients.

Provide relevant services including facilitated (warm hand-off) linkages to the multiple internal services of Catholic Charities East Bay and to appropriate community-based resources needed to help clients meet identified goals.

Minimum Requirements:

Possesses a master’s degree from an accredited professional school of social work or university preparing students for California licensure as an LCSW, MFT or psychologist.

Possesses a license as an LCSW, MFT, or psychologist or be a registered MFT Intern or MSW Associate with the Board of Behavioral Sciences.
- Cultural competence: African American, Latino, traumatized populations

Knowledge and skills of principles, models, and techniques of intensive clinical case management with a focus on impoverished and violence impacted communities.

Knowledge of laws, ethics and regulations governing mental health counseling and crisis management.

Knowledge of short-term and long-term counseling. This must include individual counseling with children, as well as family counseling or group counseling.

Knowledge of laws and regulations governing counseling sufficient to guide practice with regular supervision.

Knowledge of community resources, especially related to trauma informed practice, restorative justice or chronically traumatized communities is highly desirable.

Knowledge of Restorative Practices a plus.

Ability to drive to and from schools and community meetings in personal vehicle.

Salary: Starting at $62,400
