PRIMARY RESPONSIBILITIES:
Service Delivery 65%
- Serve as day-to-day lead for all client-facing matters related to The Way Out’s Homeless Initiative residential program
- In collaboration with officers, initiative director, and existing residential treatment staff, manage and guide support services by the organizations mission, values, and policies and procedures
- Work closely with all social service support staff to further develop and implement high-quality programming that delivers individualized counseling, intense case management, job readiness training, and life skills development
- Represent the program internally during planning meetings, board meetings, and with external stakeholders, including community groups, partners, and third-party service providers
- Oversee and facilitate resident related community-building activities both internally and externally
Case Management and Client Outcomes 35%
- Oversee client success: intake & assessment, individual service plans, ongoing case management, referrals, support groups, and other services that respond to clients' needs.
- Provide ongoing supportive and case management functions by the problems, needs, and strategies identified within the individual service plan to help the clients achieve the stated goals and objectives
- Provide support, encouragement, accountability, and regular evaluation for clients while maintaining the confidentiality
- Connect residents with needed and available community resources (internal and external), follow up with clients and agencies as appropriate to document the use/success of the referral.
- Track resident case notes, outcomes, and program deliverables
QUALIFICATIONS
- Bachelor’s Degree is required; Master’s Degree preferred
- Ability to work with government leaders, volunteers, community groups, etc.
- Ability to work with people from various backgrounds and cultures.
- Knowledge of Microsoft Windows environment, including Microsoft Office. Must be fluent in Microsoft Word.
- Self-motivated and able to motivate others.
- Excellent organizational skills, detail-oriented, accurate and precise.
- Verbal and written communication skills.
- Ability to maintain good rapport and communication with other staff members, corps personnel, and the public.
- Ability to handle multiple tasks and an ability to schedule time and resources.
- Maintain confidentiality.
- Capable of maintaining a high level of integrity.
- Work independently and as a team member.
- Strong knowledge of Salvation Army programs, practices, policies, and organization desirable.
- Must possess a current Driver's License
