School Based Clinical Case Manager

Position Summary: This position has primary responsibility for providing clinical case management services to Catholic Charities school-based trauma treatment program for youth, Experience Hope, which provides culturally-competent, trauma-informed mental health services to students and their families. Clinical Case Managers will deliver evidence-based trauma treatment to youth and families and be required to maintain fidelity to program and treatment models.

Reports to: Experience Hope Program Supervisor/Sr. Manager Youth Crisis and Family Services

Responsibilities

Leads on individual activity with clients, developing specialized interventions as needed
Conducts job responsibilities with skill and care and represents the work of the program, division, and agency to internal and external stakeholders as needed.
Learn, adhere to, and deliver evidence-based treatment models
Establish and maintain effective working relationships with school personnel, parents, and community members.
Establish and maintain effective working relationships with a variety of clients from diverse cultural backgrounds.
Conduct home visits with students and their families
Partner with school staff, CBOs, and government agencies on needs of clients and their families.
Adhere to agency and program data collection requirements

Minimum Qualifications:
Possess a master’s degree from an accredited professional school of social work or university preparing students for California licensure as an LCSW, MFT or psychologist.
Possess a license as an LCSW, MFT, or psychologist or be a registered MFT Intern or MSW Associate with the Board of Behavioral Sciences.
Experience delivering short-term and long-term counseling. This must include individual counseling, as well as family counseling or group counseling.
Experience providing culturally competent case management and mental health support to traumatized children, youth, and families.
Experience with restorative practices a plus.
Ability to drive to and from schools and community meetings in personal vehicle and possession of a valid driver’s license, proof of insurance and access to reliable incurred vehicle
Valid driver’s license, proof of insurance and access to reliable insured vehicle
TB test, Fingerprinting and background check with clearance to work with children and youth

Preferred Requirements:
Bilingual Spanish/English fluency preferred

Salary: $68,900- $75,000 per year
