Essential Functions
- Maintain a caseload of Housing Services participants.
- Complete eligibility assessment for new participants, ensure funding guidelines are met.
- Identify and assist program participants in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
- Provide case management to promote stability in housing (budgeting, self-care, developing natural supports, and independent/instrumental living skills).
- Complete Monthly Assessments regarding rent, income, assets, and expenses.
- Build and maintain relationships with local landlords and property management companies, to locate housing opportunities for housing participants.
- Complete move ins, home visits and monthly inspections of units.
- Document case management sessions with participants within 48 hours.
Other Functions
- Ensure documentation compliance with funding rules and regulations
- Enroll/update/exit participants from the Homeless Management Information System (HMIS)
- Maintain excel spreadsheet for housing participants
Qualifications
- Degree: Bachelor’s in social work or social services related field, Master’s Degree Preferred
- Experience: 2+ years

This position does require field-based work. The Manager of Rapid-Rehousing may add or remove responsibilities as occasion may require.
