Position Summary

The Housing Services Case Manager will work with a new housing department at The Stewpot, which includes Rapid Re-Housing and Rental and Utility Assistance Programs. The Housing Services Case Manager will provide supportive housing case management to individuals and families who are either literally homeless or at risk of becoming homeless. This is a grant funded position that serves to make homelessness rare, brief, and non-reoccurring. This position promotes ongoing housing stability, promotes the acquisition of income to increase self-sufficiency, and encourages self-determination of program participants.

Essential Functions

  • Maintain a caseload of Housing Services participants.
  • Complete eligibility assessment for new participants, ensure funding guidelines are met.
  • Identify and assist program participants in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
  • Provide case management to promote stability in housing (budgeting, self-care, developing natural supports, and independent/instrumental living skills).
  • Complete Monthly Assessments regarding rent, income, assets, and expenses.
  • Build and maintain relationships with local landlords and property management companies, to locate housing opportunities for housing participants.
  • Complete move ins, home visits and monthly inspections of units.
  • Document case management sessions with participants within 48 hours.

Other Functions

  • Ensure documentation compliance with funding rules and regulations
  • Enroll/update/exit participants from the Homeless Management Information System (HMIS)
  • Maintain excel spreadsheet for housing participants

Qualifications

  • Degree: Bachelor’s in social work or social services related field, Master’s Degree Preferred
  • Experience: 2+ years

Note

This position does require field-based work. The Manager of Rapid-Rehousing may add or remove responsibilities as occasion may require.
