:
The Staff Pharmacist will be responsible for delivering our mission, modeling our values, and demonstrating genuine care for our patients. The Staff Pharmacist is responsible for performing the day-to-day clinical functions of the pharmacy practice and for supporting the management, oversight, and operation of all aspects within the pharmacy including pharmacy technician staff oversight during those shifts.

Additional duties include participating in patient health data analytic initiatives aimed at optimizing appropriate product utilization and delivery of outcomes-based care. The Staff Pharmacist will collaborate with partner clinics in outcome studies, monitor data abstracted from medical health records and collected through other sources to identify compliance, trends, and patient health outcomes improvement. Furthermore, the Staff Pharmacist will work as a preceptor for Advanced Pharmacy Practice students from local Schools of Pharmacy.

Duties and Responsibilities:

  • Following established procedures, perform the duties of a clinical pharmacist.
  • Ensure that patient care meets standards of safety, effectiveness, patient rights and customer service.
  • Responsible for Staff Development where and/or when needed including:

o Stay abreast of the best practices in Pharmacy and communicate these practices to Pharmacy staff.
o Serve as a pharmacy student preceptor for local Schools of Pharmacy
o Provide leadership in departmental or health system quality improvement efforts.

  • Provide alternative therapy recommendations (as recommended by the pharmacist(s)) to PCPs for review when a particular medication becomes unobtainable as not to cause an interruption in therapy.
  • Serve as a resource or consultant to healthcare professionals in the care of their patients.
  • Provide therapy monitoring for the purpose of evaluating and optimizing patient outcomes, and drug utilization via access to data obtained at both the clinic(s) and SVdP Pharmacy.
  • Evaluate that selected treatment plans are optimal when poor outcomes necessitate such intervention.
  • Participate in the intervention, adverse drug reaction and medication variance reporting systems.
  • Oversee the management and reporting of clinical utilization and patient health collaborate studies. Develop and present reports as needed related to these and other quality initiatives.
  • Act as an outcomes of care liaison between SVdP Pharmacy and partner clinics by collecting objective, pre-treatment baseline parameters that will continuously be compared throughout the course of treatment to ensure optimal outcomes are achieved.
  • Provide implementation planning to patients, PCPs, and SVdP Pharmacy to ensure that patients not only are receiving appropriate treatment, but to also ensure that the selected treatment options are both obtainable and sustainable by SVdP Pharmacy throughout the course of therapy.
  • Work with the Patient Advocacy Team to ensure that the Patient Intake Process is efficient and fully compliant.

Job Requirements
Knowledge, Skills and Abilities:

  • Demonstrated knowledge of pharmacy professional practices and regulatory requirements.
  • Solid understanding of pharmacy workflow management, inventory management and loss prevention procedures.
  • Able to keep confidential information regarding patients, team members.
  • Strong organizational, interpersonal and written and verbal communication skills required.
  • Excellent computer software skills (including Microsoft Office (Outlook, Word, Excel)), pharmacy systems and database management.
  • Knowledge and experience with electronic health records.
  • Strong clinical assessment, data management and interpretation skills required.
  • Must demonstrate the ability to handle multiple projects simultaneously, prioritize tasks based on organizational need and judgement.
  • Must be flexible and self-motivated.

Minimum Qualifications:

  • Bachelor’s degree in Pharmacy required, PharmD preferred.
  • Currently holds license as a Pharmacist and Preceptor for the state of Texas.
  • Passionate about quality customer service and care
  • Experience in medical record reviews, quality improvement and/or utilization management preferred.
  • Experience in health outcomes measurement/management preferred.
  • Ability to work with people of diverse backgrounds in a faith-based setting.
  • Bilingual (Spanish and English) strongly preferred.

Physical Requirements:

  • Ability to bend, and/or carry light items up to 35 lbs.
  • Ability to walk, stand, for periods of 3 hours or greater
  • Ability to drive an automobile.
