Job Title: Connection Center Resource Navigator Case Manager

Corps/Department: Divisional HQ – Connection Center

Reports to: Connection Center Program Director

FLSA Status: Regular, Part-Time (up to 29 hours per week), Non-Exempt. This position pays at $22.00 per hour and includes both vacation and sick time-off accruals.

Scope of Position:
The Resource Navigator Case Worker position supports the provision of Coordinated Entry of social services in Colorado by providing information, referral, intake assessment, and emergency casework services over the phone, via email, video chat, and/or in person. The Resource Navigator Case Worker is responsible for working in conjunction with Housing Now and the Coordinated Entry Specialist, to provide tailored referrals to the appropriate agency based on individual programs criteria. The Resource Navigator Case Worker will provide case management services using homeless diversion and prevention strategies. The Resource Navigator Case Worker will also be responsible for using HMIS and Salvation Army databases to document assistances, referrals, and to track assisted households monthly, for at least one year. Provides easy access to tailored solutions for families in Metro Denver that are experiencing homelessness, are at risk of experiencing homelessness, or who are precariously housed, to promote long term housing stability.

Essential Functions:
Answers calls for information regarding The Salvation Army’s assistance programs and other community resources
Assess needs and identify resources to establish individual case plan
Connect qualified candidates with their appropriate partner agency
Provide tailored case management and financial assistance when appropriate
Documents calls accurately utilizing a computerized internal database
Record and track assistances in HMIS according to HUD standards
Completing all required assessments with households to determine program eligibility
Use active and compassionate communication skills
Other duties as assigned

Education and Work Experience:
Bachelors degree in social work or a related field is required
Master’s degree in social work or a related field is preferred
Required one-year experience with providing casework services
Experience working with individuals from a variety of backgrounds and educational levels required Customer service experience

Knowledge, Skills and Abilities Required:
Have a passion for The Salvation Army mission
Demonstrated history of providing professional, effective case management with households
Desire to serve others by building on their strengths
Competent in working independently within clear parameters
Excellent customer service skills
Able to creatively problem-solve and think on their feet
Excellent attention to detail skills
Strong communication and people skills in person, over the phone, and via email
Experience with HMIS and Clarity database systems, preferred
Previous experience working with populations experiencing homelessness or working with vulnerable populations, preferred
Trauma Informed Care and Crisis Prevention Intervention knowledge, preferred
Minimally 21 years of age; possessing a valid Colorado Driver’s License

Software-related skills: Microsoft Word and Excel required, Microsoft Outlook, Working knowledge of integrated database applications and ability to use new software programs with basic training.

Physical Requirements: Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or continuous basis. Ability to grasp, push, pull, and reach overhead. Ability to operate telephone. Ability to lift up to 25 pounds. Ability to access and produce information from the computer. Ability to understand written information.
