Duties & Responsibilities:
**Intake and Assessments:

  • Within 24 hours will meet with minor and introduce themselves Conduct initial intake assessment within 24 hours and enter in the UAC Portal.
  • Develop goal-oriented service plans and initiate the Individual Service Plan in the UAC Portal within five days
  • Identify and initiate communication with the potential sponsor
  • Once sponsor is identified, complete sponsor assessment within five days.
  • Complete the Case Review after 30 Days or if significant changes occur after five days or arrival into the program.

Service Delivery:

  • Gather sponsor information and assess whether the potential sponsor is a suitable sponsor who can safely provide for the physical and mental well-being of the child or youth.
  • Provide direct assistance on completing the sponsor application packet and ensuring provision of supporting documentation
  • Keep the sponsor informed of the child’s progress and current functioning; provide the sponsor with detailed information about the child’s needs in order to fully assess the sponsor’s ability to provide care and services, including completing a sponsor care plan, when necessary;
  • Meets minimally one time per week with each child on their caseload and provides status updates on the child’s case and provision of services.
  • Attends weekly staffing and provides weekly status updates to the unaccompanied alien child’s Case Coordinator and ORR/FFS on the progress in achieving a safe and timely release with family members as well as potential challenges that may delay a release.
  • Makes recommendations for Home Study, referral to Foster Care, Follow-up services, return to home country, or release recommendation.
  • Informs other stakeholders of the progress of a minor’s case, including notification that a minor may not have a potential sponsor, and any final release decisions.
  • Collaborates with the ORR/FFS and Case Coordinator and work with law enforcement officials regarding a minor pending release, if there are outstanding criminal charges or other issues.
  • Explore and secure resources for post-18 planning and safety planning.
  • Participates in staffing to determine a minor’s Supervision needs, as well as step up services.

Documentation, Monitoring and Reporting:

  • Maintain proper documentation by filing and uploading weekly the following documents that are included but no limited to: progress notes, and case summaries, case reviews and, change of venue/change of address, release requests, and Individual Service Plan, Family Reunification Packets, Home Studies.
  • Ensure the timely and accurate filing and uploading of documents in the portal system according to timeframes specified by ORR, Catholic Charities and COA Accreditation.

Discharge:

  • Ensure the completion of the discharge process by keeping all the necessary documents in record and notifications to the different agencies submitted.
  • Submits timely and accurate Reunification packet to GDIT Case Coordinator.
  • Coordinates the safe and timely release of the minor with approved sponsor.
  • Works towards meeting a weekly 20% discharge rate.

Risk Management and Quality Assurance:

  • Performs PQI functions as assigned by Supervisor.
  • Participates monthly in the fire drill
  • Timely and accurately submit Unusual Incident Reports and include all relevant documentation required by regulatory bodies.
  • Must be available to work beyond your regular schedule in crisis situations or if there is a declared agency emergency.

Additional Duties:

  • Participates in all related trainings, in-services, and staff meetings as required by COA and funding /licensing sources.
  • Incumbent will comply with all policies, procedures and requirements necessary to perform the functions of this position.
  • Ability to work a flexible schedule including early mornings, late evenings and some weekends.
  • Consistently demonstrate the ability to respond to changing situations in a flexible manner in order to meet current program needs, such as reprioritizing work as necessary.
  • On call 24 hours 7 days a week unless otherwise specified by supervisor.
  • Performs other duties as assigned by supervisor.

Educational & Experience Requirements:

  • Bachelor’s Degree in the behavioral sciences, human services or social service fields.
  • Child Welfare and or Case Management experience is strongly encouraged.
  • Bilingual English and Spanish Required

Bachelors or better in Human Services.
