Duties & Responsibilities
Screening & Intake

  • Screen potential consumers within 24 hours of referral.
  • Provide for placement on a waiting list if consumer cannot be intake.
  • Consumer who cannot be served or cannot be served promptly are referred, or connected to appropriate resources.
  • Enter client data into agency’s internal data base (Collaborate) within 72 hours of intake.

Assessment

  • Use standardized criteria and assessment instruments to match the needs of the consumer and their families with the level and intensity of available services.
  • Complete assessments within 7-10 days of intake.

Service Planning & Monitoring

  • Develop a service plan within 30 days of intake.
  • Develop a service plan with the full participation of the consumer receiving services.
  • Service plans are based on the assessment and include:
  • Agreed upon goals.
  • Desired outcomes.
  • Timeframes for achieving outcomes.
  • Services and supports to be provided.
  • The service recipient’s signature.
  • Review and update each consumer’s service plan quarterly.

Therapeutic and Educational Interventions

  • Engage and motivate consumers and families by providing weekly or biweekly therapy.
  • Provide group, individual, couple and family therapy sessions as assigned.
  • Chart weekly in progress notes.

Operational Job Function

  • Maintain compliance to all Agency, Government, Funding Sources, COA and Catholic Charities requirements regarding clients’ confidentiality (charts/files).
  • Develops an Aftercare Plan within 30 days of intake.

General Job Function

  • Attends and participates in staff meetings.
  • Weekly review with supervisor of caseload and clinical issues.
  • Attends case record review monthly.

Case Closing:

  • Case closing begins at intake and culminates in the consumer completing the agreed upon service goals.
  • Upon closing a case all collaborating service providers are notified if applicable.
  • Consumers discharged prior to completing the program are linked by the organization to appropriate services.

Aftercare & Follow-Up:

  • With the permission of the consumer, follow-up by phone post discharge

Outreach:

  • On a regular basis or as directed by supervisor incumbent will conduct outreach visits to nearby parishes, agencies or other relevant entities.

Additional Duties

  • Incumbent will comply with all policies, procedures and requirements necessary to perform the functions of this position.
  • Perform additional responsibilities as required by supervisor.

Physical Demands

  • Ability to travel to all centers and meetings/trainings locations.
  • The work is usually sedentary, but may require long periods of standing, walking and /or bending and some light lifting. The work is mostly performed in a secure office setting.

Educational & Experiential Requirements:

  • Licensed or License Eligible in Social Work, Marriage and Family Therapy or Mental Health Counseling.
  • Bilingual English/Spanish required.
