POSITION SUMMARY:
This is an exempt position that will be responsible for the administrative functions of a residential program, including but not limited to shelter services, programmatic functions, and ensuring supports and services of individuals served. The Program Coordinator provides support to the Senior Director and is a second liaison with funders. This position works in conjunction with other positions at the program using a multidisciplinary approach for the provision of services. The program operates twenty-four hours a day, seven days a week. All activities within this position are completed in accordance with agency policy and procedures, Department of Children and Families (DCF), Thriving Minds South Florida Behavioral Health Network, Inc., federal, state and local laws, accreditation standards, other regulatory guidelines and tenets of the Catholic Faith. The Incumbent in this position demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.

Duties & Responsibilities:

Shelter Services:

  • Create and maintains a caring, safe, and clean service environment for Program.
  • Ensures intake and discharge processes of clients as required by policies and procedures.
  • Ensure that all the needs of our consumers are recognized and are met in a coordinated manner and that there are minimal gaps in services provided.
  • Ensures staff oversees and manages the care, safety, and well-being of each individuals served.
  • Actively participate in interdisciplinary team meetings.
  • Oversees regular inspection of facility and grounds.
  • Oversees programs’ multi-service components assuring the coordination and delivery of all services in a teamwork environment.
  • Meet with administrative team to discuss opening and closing of beds and units needed to maximize contract.
  • Provide SOAR services (SSI/SSDI Outreach, Access), and Recovery in coordination with the interdisciplinary team to eligible consumers.
  • Provide supervision for consumers when attending court or other necessary appointments.

Programmatic:

  • Provide information to those in the community requesting information regarding admission process.
  • Gives priority to consumer that meet the priority criteria and with urgent needs or emergency situations.
  • Conduct pre-screen for potential consumers within 24 hours of referral and prescreening details of consumers pending admission at staffing.
  • Coordinate and facilitate intakes of consumers, including assessment of possible case management needs.
  • Consumers who cannot be served or cannot be served promptly are referred or connected to appropriate resources for the provision of interim services.
  • Assists in the formulation and implementation of policies and procedures as per regulatory requirements and makes revisions as needed.
  • Ensures that all files include documentation in compliance with funders and Catholic Charities Policies and Procedures requirements.
  • Adheres to the dress code and wears appropriate clothing and follows safety protocols per OSHA and Catholic Charities when conducting job functions.

Human Resource Management:

  • Provides supervision, direction, and support to residential and program staff and ensures 24-hour coverage at all times and adherence to medication protocols.
  • Ensures orientation and training is provided to staff according to regulatory body requirements.
  • Schedules and conducts monthly staff meetings.
  • Ensures that staff receives required training and professional development within the specified time frames.
  • Provides on-going supervision to each staff under the incumbent’s supervision.
  • Conducts performance evaluations and/or improvement plans for all staff directly under his/her direct supervision.

Risk Management and Quality Assurance:

  • Completes and submits timely and accurate Significant Incident Reports, CIP Reporting and any other required documentation as per Catholic Charities guidelines, state licensing requirements, funder requirements and COA Standards.
  • Responsible for following up on all incidents reported timely and accurately.
  • Participates monthly in the fire drill
  • Annually assists in completing and submitting Tactical Plans.
  • Annually assists in the distribution, collection, and aggregation of Consumer Satisfaction Survey.
  • Participates in the Performance Quality Improvement process.
  • Monthly, Quarterly, and Annually track services for the completion of Quality Assurance Reports.
  • Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.

Other Duties:

  • Oversees the program operations in the absence of the Senior Director.
  • Performs other duties as assigned by supervisor.
  • Must be available to work beyond your regular schedule in crisis situations or if there is a declared agency emergency. On call 24 hours 7 days a week unless otherwise specified by supervisor

Physical Demands:

  • Must possess a valid Florida Driver’s License
  • The work is usually sedentary, but may require long periods of standing, walking and /or bending and some light fitting. The work is mostly performed in a secure office setting.
  • Ability to detect unsafe situations by sight or sound and react quickly to intervene.

Educational & Experiential Requirements:

  • Master’s Degree in social work or related field.
  • 1 year of related work experience required.
  • 1 year of supervisory experience required.
  • SOAR (SSI/SSDI Outreach, Access, and Recoverycertified or completes certification within 6 months from date of hire.
  • ServSafe Food Manager Certification or completes certification within 90 days from date of hire.
