Summary

The Targeted Case Manager will provide support and guidance and the necessary tools to the recipients of Servant's Heart Ministry to accomplish increased self-sufficiency and help to maintain a healthy lifestyle: physically, emotionally, and spiritually. This is a part-time, non-exempt regular position located in Orlando, FL. Compensation is commensurate with skills and experience.

 

Qualifications

 

  • Bachelor's Degree in Biblical Counseling, Social Work, or Human Services recommended.
  • A solid Bible-based Christian faith, compatible with SHM's statement of faith.
  • Current and active fellowship/membership in a local church.
  • Prior experience as a Case Manager/Social Worker/Counselor.
  • Bi-lingual Preferred.

Essential Duties and Responsibilities

 

  • Oversee all SHM recipients intake and case management.
  • Conduct intake, assess client's needs and make referrals to appropriate community resources.
  • Conduct case management meetings with SHM recipients for financial management, accountability, resource needs, and transition planning.
  • Provide advocacy, monitoring, follow-up, and maintenance of recipient records and documentation.
  • Develop linkages with other programs and organizations that provide direct and indirect services.
  • Maintain accurate recipient documentation and other pertinent paperwork/reports.
  • Assist recipients in achieving goals as outlined in their development.
  • Accurately complete job assignments, agency forms, and written documents within assigned deadlines.
  • Act as a hub for all information about recipients kept by Servant's Heart Ministry staff.
  • Be the liaison to the Executive Director and Community Partners concerning recipient development.
  • Maintain contact and facilitate meetings with the Executive Director to inform program success and challenges.
  • Accurately represent Servant's Heart Ministry's mission and vision.

 

Computer Skills

 

  • Software Must be proficient in Windows, Microsoft Word, and Excel.
  • Essential office equipment, such as computers, copiers, and printers.
