Case Manager - Prenatal/Parenting Educator

MAJOR RESPONSIBILITIES:
Develop, recruit and facilitate prenatal and parenting educative and supportive programs by utilizing evidenced based curricula such as Parents as Teachers, Partners for a Healthy Baby, Parent Caf and Nurturing Families while embracing a Family Strengthening framework through-out service delivery. Develop and facilitate pregnancy and parenting classes for teens, women, and men to enhance education. Provide some evening pregnancy/parenting at a CSS site or community partner site.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provide educational services to pregnant women identified as having stressors that could potentially interfere with a healthy full term birth
- Responds directly to all incoming calls/walk-ins requesting pregnancy/parenting services
- Provide comprehensive services, linkage to appropriate community resources
- Provide weekly pregnancy education classes at assigned WIC offices
- Responsible for effectively organizing and managing the Diaper Bank, Baby Boutique
- Work collaboratively with other community organizations in providing programming to address the needs of individuals/families experiencing prenatal and/or parenting needs;
- Maintain weekly documentation of services and maintain participant records in an organized manner utilizing the Harmony database or other assigned computer database;
- Provide monthly/quarterly service reports of group statistics and outcomes
- Complete and submit all Real Alternative paperwork into the system weekly and maintain organized confidential files for eligible participants participating in the program;
- Actively participate in Real Alternatives and all contractual funded programs' meetings/conferences/trainings;
- Work collaboratively with Parishes in providing programming to address the needs of individuals/families experiencing pregnancy and/or parenting needs at the Parish level.
- Participate in monthly staff meetings, weekly supervision, and CSS trainings as well as required trainings of funders;
- Assist in program development and innovation
- Make homes visits as necessary;
- Assist with planning, organizing and recruitment for all program functions
- Submit a weekly schedule of activities to supervisor;
- Assist with meal prep for participants in classes;
- Provide Spanish/English translation services (if bilingual)
- Other tasks and duties assigned by the supervisor
REPRESENTATIVE KNOWLEDGE, SKILLS, AND ABILITIES
- Education: Bachelor's degree in Social Work (BSW)
- Experience: 2 years related experience and/or training or equivalent combination of education and experience. Bilingual Spanish/English preferred
- Physical requirements: minor lifting, less than 10 pounds
- Knowledge of and orientation of philosophy of the Agency's policies as presented in Mission Statement.
- Knowledge of and commitment to Catholic values and teachings.
- Knowledge of crisis intervention techniques, trauma informed care, child abuse reporting, and de-escalation techniques
- Skill in working with clients with mental health diagnosis
- Strong knowledge of community resources.
- Ability to maintain a professional partnership with clients and community organizations at all time
- Culturally competent and inclusive of all members of the community
- Proficient in computer software (Microsoft Office) usage.
- Valid PA driver's license and driver in good standing.
- Requires traveling to other Catholic Social Services; locations as well as local agencies;
- Some evening/weekend hours' maybe required.
