DUTIES AND RESPONSIBILITIES:
1. Provides Single Family Consultations

Establishes and maintains on-going relationships with families of diverse backgrounds.
Provides supportive counseling, psychoeducation, and skills coaching in the areas of mental health awareness, communication, advocacy, and problem solving.
Visits families at a frequency consistent with their assessed level of concern, and status of family’s access to mental health and or crisis need.
Develops and implements appropriate assessment tools of service preferences, level of concerns, family strengths, and identified family goals.
Records status of family’s access to mental health services, community resources.
Directs assistance in resource acquisition.
Follow-up with timely manner to all new referrals and active clients.
Provide consultation during normal workday hours, and nights and weekends as needed.

2. Facilitates Family Support Groups

Conducts family support groups to ensure mutual exchange of information which empower families with a recovery process.
Plans, develops, and presents psychoeducational lecture series, according to family needs and resources.
Provides groups with a time-limited or ongoing basis, with purpose to meet family needs.
Enhance families overall functioning with facilitator supportive group counseling.
Facilitate groups with couching skills, skilled group processing and group cohesion.
Groups are offered with night and weekend hours.

4. Facilitates Respite Care Services

Provides, for non-emergency, respite services with the ill family member, either in or out of the home.
Conducts respite groups for the purpose of families uniting in planned activities together.
Where appropriate, coordinates with appropriate programs to facilitate overnight respite services for the ill family member.
5. Promotes Advocacy
Advocates on behalf of families when needed services are denied or delayed.
Identifies new services, resource gaps, and/or problems of service delivery.
Directs families to NAMI Burlington and their Family-to-Family series, as well as other self-help organizations.
Attends NAMI support groups meetings bi-monthly in the evenings, to offer support to families.

6. Documents Service Delivery

Maintains appropriate counselor caseload, total daily client services, monthly referrals, collateral services, and groups.
Completes accurate assessments, treatment plans, progress notes, and appropriate documents pertaining to all record keeping of service delivery.
Submits an accurate daily activity log within 48 hours.
Documents all records electronically within 24 hours.

7. Continues Education

Expands knowledge of available community resources and services.
Attends IFSS/NAMI trainings and other appropriate trainings, to enhance knowledge and skill sets.

8. Maintains Ethical Standards and Code of Conduct

Displays commitment to the Mission Statement of Catholic Charities and the philosophy of IFSS.
Adheres to codes of conduct governing family/family and therapist relationships as established through Catholic Charities and other applicable codes of conduct.
