Job Summary:
This position will provide primary health care educational and support services to SMI consumers who are enrolled in RIST.
Responsibilities:
Orient RIST consumers to basic principles of primary care management.
Consult with RIST consumers and family members to develop and monitor wellness assessments.
Assist in care coordination with the project RN to ensure access to primary health care.
Help coordinate and ensure consumer access to the array of wellness and recovery education and self-help programs.
Participate in regularly scheduled team meetings.
Escort consumers to medical appointments and lab appointments, as necessary
