The Outreach worker will meet with all prospective clients to assess both need and eligibility, and will serve as the primary connection for veterans to access SSVF services. This position will require a significant degree of autonomy, as well as the ability to work with a team. Responsibilities · Outreach very low-income Veteran families for the SSVF program. Conduct outreach in coordination with community service providers. · Assist those who are camping or are living in non-habitable situations who may qualify for SSVF. · Build relationships with those individuals with the goal of assisting them in obtaining permanent housing. · Engage in meaningful relationships with SSVF program participants, encouraging clients to break the cycle of homelessness. · Engage in crisis intervention strategies with SSVF program participants when appropriate. · Conduct Intake responsibilities for the SSVF program. Maintaining program participant records. · Assist in development of positive working relationships with other agencies serving SSVF clients. · Build and maintain a relationships with local institutions (jails, prisons, mental health facilities, hospitals, VA Medical Center, VA Outreach, etc.) so as to assess very low-income Veteran families for the SSVF program. · Collect required SSVF intake program documentation and data for accurate assessment and intake of very low-income Veteran families and program evaluation.

· Assist program participants in completing SSVF program intake forms.
Qualifications and Education Requirements · At least 2 years’ experience in homeless services or related field · Bachelor’s degree in Social Work or related field · Excellent verbal and written communication skills · Computer experience and skills in word processing and databases required · Team player who has superior client skills as well as capacity to engage in critical thinking and analysis of program and service delivery · Flexibility: ability to adjust to the diverse needs of clients · Excellent attention to administrative detail · Highly responsible and able to work with independence · Ability to positively engage with prospective clients, other staff, and community partners · Strong public speaking ability Additional Notes This job description does not list all of our job duties. Job Type: Full-time

Pay: $37,000.00 - $40,000.00 per year
