Primary responsibilities of this position include:
- Manage a caseload of approximately 98 senior households at two independent living apartment communities in South Philadelphia.
- Assess all new residents for resident's key strengths and barriers to successful aging in place, including physical and behavioral health/functioning, health insurance, finances, safety, fall risk, transportation, food security, legal issues, and social support.
- Create service plan of interventions that are culturally sensitive for the individualized needs.
- Keep resident files up to date in the social service database including case notes, diagnoses, key contacts, assessments, and care plans.
- Navigate access to services and advocate to ensure any roadblocks to service are overcome.
- Create and maintain a community atmosphere that addresses holistic needs of the residents through in-house events and coordination with outside entities. Such events and gatherings requiring planning may include: holiday and birthday parties, educational presentations on health care needs, or on-site clinics such as flu shot clinics or senior legal clinics.
- Maintain statistics on social work interventions and group participation.
- Assist with responding to external phone calls for information and resources.
Position Requirements:
- Master's degree from an accredited university in Social Work, Gerontology or related field required.
- Minimum three (3) years' experience working with seniors.
- Knowledge of federal and state benefits programs and their eligibility requirements.
